Participant Forms

Thank you for your interest in the Mayo Clinic Biobank. If you know your Mayo Clinic number, you are welcome to print out our documents, complete them, and mail them in (or drop them off if you are on the downtown Rochester Mayo Campus).

If you do not know your Mayo Clinic number, please call registration at 507-284-2421 to retrieve it before you proceed, or you can call or email the Biobank to request a fully labeled packet be sent to you.

If you print from our web site, please be sure to do the following:

Consent document

• Read it carefully and please contact us with any questions or concerns
• Ensure the status of the 2 boxes on page 4 and page 10.
• Sign your name and date the last page. Do not detach this last page.
• Write your full name, Mayo clinic ID, and date of birth on the first page

• Select your preferred gift(s)
• Write your full name, Mayo clinic ID, and date of birth on the top
