Federalwide Assurance (FWA)

Mayo Clinic's IRB has a Federalwide Assurance (FWA) on file with Office for Human Research Protections (OHRP). This Assurance Agreement is between Mayo and the OHRP, which acts on behalf of the Secretary of the Department of Health and Human Services.

The FWA outlines the principles and guidelines that govern the Mayo Clinic, faculty and staff, in the protection of the rights and welfare of the participants in the research conducted at or sponsored by Mayo Clinic.

The Federalwide Assurance number for Mayo Clinic is FWA00005001 - valid through February 14, 2014; the Department of Defense Addendum number is F50420 - valid through January 21, 2014.
