Population Health Science Program

The Population Health Science Program envisions optimal health and well-being for community members by leveraging existing health care delivery systems and community resources.

Researchers within the program work to identify medical conditions posing the greatest risk for adverse health outcomes in the community and develop interventions and strategies to address them.

The Population Health Science Program includes:

• Southeast Minnesota Beacon Community
• Rochester Epidemiology Project (REP)
• Community care coordination projects, such as Diamond, Emerald and the Institute for Clinical Systems Improvement
• Projects involving telemedicine, health promotion and medical homes
